Certificate of Immunization records
Clients can obtain copies of their immunization records from Cobb & Douglas Public Health for $5.00 per record. Records will be retrieved from the GA Registry of Immunizations Transactions & Services (GRITS). If you have out-of-state records, please see instructions for transfers below.
Private physicians can also provide copies. Fees may apply.

Out-of-State or Out-of-Country Immunization Record Transfers
For children who have vaccine records from out-of-state or out-of-country, parents should submit their vaccine records online for transfer to a Georgia form. To transfer vaccine records from out-of-state or out-of-country, please submit the requested information by clicking the button below. Please allow 2-3 business days for staff to review your submission. (Note: Due to high volume during the Back to School period, please allow up to 5 business days for completion of transfer.) Following completion, we will contact you to discuss your records and assist in scheduling an appointment, if needed. *Please note, there is a $10 charge for out-of-state transfers and a $25 charge for out-of-country transfers.
