Through our Outreach Services, CDPH offers a wide-range of exceptional, cost-effective health services for both adults and children.
The nurses, technicians, and educators on our team are some of the most caring and respected in their fields, striving daily to lead others and raise the bar for compassion, confidentiality, professionalism and attentive customer care.
The Outreach Team travels to various locations in Cobb and Douglas county to provide various vaccines, as well as biometric screenings, hearing/vision/dental and nutrition screenings, health education, homebound visits and wellness checks to create and sustain healthy communities. We also offer an extensive referral network to guide clients to any additional medical or social service needed.
