Out-of-State or Out-of-Country Immunization Record Transfers

For children who have vaccine records from out-of-state or out-of-country, parents should submit their vaccine records online for transfer to a Georgia form. To transfer vaccine records from out-of-state or out-of-country, please submit the requested information by clicking the button below. Please allow 2-3 business days for staff to review your submission. Following completion, we will contact you to discuss your records and assist in scheduling an appointment, if needed. *Please note, there is a $10 charge for out-of-state transfers and a $25 charge for out-of-country transfers.
