*Please Note: CDPH is now offering $100 gift card incentives at our Marietta, Smyrna, Douglas, and Acworth-Kennesaw Health Centers and at CDPH outreach events for those who receive either their COVID primary dose, COVID secondary dose or COVID booster. The incentive is available for ages 12 years and older while supplies last. Please note: BOH and DPH employees are not eligible for this incentive.
