Cobb & Douglas Public Health (CDPH) is offering a COVID-19 vaccine incentive during the month of August 2021. This drawing is for anyone vaccinated (COVID-19 1st, 2nd or 3rd dose) at a Cobb & Douglas Public Health Clinic (See Locations) or from a CDPH/CORE Outreach event (See Sites) between August 16, 2021 and August 31,2021.

Enter below to be registered to win an Atlanta Braves package that includes tickets to the September 8th Home Game vs. the Washington Nationals. Package includes 4 lower level tickets, 2 parking passes, $60 food/beverage credit. Game location: Truist Park

By registering, you give consent to have your vaccination status verified through the Georgia Immunization Registry (GRITS) if your name is chosen from a random drawing of all entrants received by 11:59 p.m. on August 31, 2021. Only verified individuals will be eligible to win the ticket package. If you are 17 years of age or under, by registering for this prize, you confirm that you have consent from a parent or guardian to do so.

Eligibility:

  • All participants ages 18 and up who received a COVID-19 vaccination from any Cobb & Douglas Public Health Clinic location (i.e., Marietta, Smyrna, Acworth-Kennesaw or Douglas), or satellite COVID-19 vaccination events provided by CDPH or CORE within Cobb & Douglas Counties between August 16, 2021 and August 31, 2021, are eligible to win.
  • Individuals 12-17 years of age who have also been vaccinated during the same time period and through the same sites as noted above are also eligible to enter with consent from a parent or guardian. A signed consent form will be required if an individual 17 years of age or under wins.
  • CDPH and CORE employees and their family members who meet the above requirements are eligible to enter.
  • No purchase, payment or financial contribution is necessary to participate in the drawing and purchase, payment or financial contribution will not increase odds of winning. drawing is void where prohibited or restricted by law.
